1-2A Violation of Code: [511-6-1.03(1)(a)-(c)1,4,5,6-14,15, 16] Observed PIC unable to demonstrate knowledge of rules and regulations as it relates to food safety. PIC did not have knowledge of requirement to have sanitizer test strips (no strips were found in facility), was unable to locate a food thermometer or answer raw meat procedural cooking questions. PIC could not accurately answer questions about date marking of TCS foods. CA: PIC and all food service employees needs to be re-trained by serve safe manager.  Repeat Violation.
1-2B Violation of Code: [511-6-1-.03(3)(d)] Observed CFSM unable to assess training needs of food service employees. CA: All food service employees need to be properly re-trained. Facility must provide evidence that a current employee is enrolled in a certified food safety manager class within 30 days, or hire on someone who is already certified.  New Violation. Correct By: 11/28/2021
2-2A Violation of Code: [511-6-1.03(4)(a)(b)(c)(e)(f)] Observed PIC unable tp provide documentation or otherwise satisfactorily demonstrate during the inspection, that all food employees and conditional employees are informed of their responsibility to report to management information about their health and activities as it relates to diseases that are transmissible through food. CA: Discussed with PIC. All food service employees will read the employee health policy and sign a conditional reporting agreement.  New Violation. Correct By: 11/05/2021
4-1A Violation of Code: [511-6-1.04(4)(c)1(i)(ii)(iii)(v)(vi)(vii)(viii)] Observed improper vertical and horizontal separation of raw animal foods and fully cooked foods. Discussed with PIC. Freezer was re-organized so that all foods were separated properly. Risk control plan will be delivered.  Corrected On-Site.  Repeat Violation.
6-1A Violation of Code: [511-6-1.04(6)(f)] Observed potentially hazardous TCS food items (honey mustard dressing) stored at ambient temperature in dry storage (72 degrees F) when labelling indicates that refrigeration is required. CA: Discussed with PIC, all items were discarded.  Corrected On-Site.  New Violation.
6-2 Violation of Code: [511-6-1.04(6)(g)] Observed opened package of sliced cheese and diced cheese with no date mark labelled on it and an opened package of potato salad held past the date marked expiration. CA: Discussed with PIC. Items were discarded.  Corrected On-Site.  New Violation.
10D Violation of Code: [511-6-1.04(8)(b)] Observed potentially hazardous TCS food (potato salad) held past printed expiration date. CA: Discussed with PIC. Potato salad was discarded.  Corrected On-Site.  New Violation.
Remarks Discussed moving all containers of chemicals and sanitizers at least 6 inches off the floor.
