1-2A Violation of Code: [511-6-1.03(1)(a)-(c)1,4,5,6-14,15, 16] PIC unable to demonstrate knowledge of foodborne illness risk factors (supervision, employee heath, protection from contamination, holding of TCS foods and date marking of TCS foods). All food service employees need to be re-trained.  New Violation.
1-2B Violation of Code: [511-6-1-.03(3)(d)] CFSM not performing duties by ensuring all employees are properly trained and following food safety practices in establishment to eliminate the risk to the public. Personnel must be re-trained on food safety regulations to ensure compliance with code. This will be re-evaluated on follow-up inspection.  New Violation.
1-2B Violation of Code: [511-6-1.03(3)(a)] No managers or supervisors employed at the facility have the food safety manager certification. It has been longer than 30 days since the previous CFSM has ceased to work at the facility. CA: A CFSM must be hired or transferred from another location, OR a current manager at the facility must enroll in a food safety manager class by 11/12/19, and complete the class within 30 days. Documentation of class enrollment must be provided to the health department by 11/12/2019.  New Violation. Correct By: 11/12/2019
2-2A Violation of Code: [511-6-1.03(4)(a)(b)(c)(e)(f)] The PIC could not provide documentation or otherwise satisfactorily demonstrate during the inspection, that all food employees and conditional employees are informed of their responsibility to report to management information about their health and activities as it relates to diseases that are transmissible through food. CA: all current food service employees will read and sign the conditional employee reporting agreement.  New Violation. Correct By: 11/12/2019
2-2B Observed personal drink in a reusable mug on the sanitize portion of the three compartment sink. CA: Drink was relocated to a separate employee area. PIC informed that employee drinks stored in the kitchen must be in a single use cup with lid and straw and stored in a designated employee area.  Corrected On-Site.  New Violation.
2-2E Violation of Code: [511-6-1.03(6)] Bodily fluid cleanup kit was not present in facility. CA: PIC will order or build a clean up kit that meets the guidelines of their bodily fluid clean up procedure.  New Violation. Correct By: 11/12/2019
4-1A Violation of Code: [511-6-1.04(4)(c)1(i)(ii)(iii)(v)(vi)(vii)(viii)] Observed improper vertical and horizontal separation of raw animal foods and ready to eat foods in freezer. Opened packages of raw pork and raw sausage patties were stored over top of fully cooked eggs and fully cooked chicken patties. Raw pork was stored on the same shelf as opened packages of fully cooked chicken. Cooked pork was stored directly on top of raw pork. CA: Raw items were relocated to the bottom portion of the freezer.  Corrected On-Site.  New Violation.
6-1A Violation of Code: [511-6-1.04(6)(f)] Observed potentially hazardous TCS food cold held at greater than 41 degrees Fahrenheit. Observed fruit on bar (diced melons) at 49 degrees Fahrenheit and shredded cheese on bar at 65 degrees Fahrenheit. CA: PIC discarded fruit, discussed using an ice bath or time as a public health control in the future.  Corrected On-Site.  New Violation.
6-2 Violation of Code: [511-6-1.04(6)(g)] Observed potentially hazardous TCS foods (milk, shredded cheese, yogurt) in the reach-in cooler that had been opened and not date marked. CA: Dates were added to these items, discussed with PIC that all TCS items that are opened/prepped and held longer than 24 hours must be date marked.
