2-1B Violation of Code: [511-6-1.03(5)(c)] Observed employee switch tasks from working with raw chicken to fully cooked chicken without first washing hands. CA: Discussed with PIC. Training will be provided. All parts of chicken were heated to 165°F.  Corrected On-Site.  New Violation.
4-1A Violation of Code: [511-6-1.04(4)(c)1(i)(ii)(iii)(v)(vi)(vii)(viii)] Observed liquid biscuit dressing stored with raw chicken in walk in cooler. CA: discussed separation. Training will be provided. Items were moved.  Corrected On-Site.  New Violation.
4-2B Violation of Code: [511-6-1.05(6)(n)] Observed multiple sanitizer buckets not at manufacturer recommended concentration level (observed: 0ppm in all buckets). CA: sanitizer buckets were changed.  Corrected On-Site.  New Violation.
5-1A Violation of Code: [511-6-1.04(5)(a)] Observed chicken cooked to 145F, not the required 165°F. CA: Employee placed chicken back into fryer and cooked to 165F.  Corrected On-Site.  New Violation.
6-1A Violation of Code: [511-6-1.04(6)(f)] Observed potentially hazardous raw chicken, coleslaw slurry, and shredded cabbage/slaw mix not stored at 41°F or below. CA: Items were placed in walk-in cooler to rapidly cool to 41F since items were out of temperature for less than 4 hours.  Corrected On-Site.  New Violation.
6-1B Violation of Code: [511-6-1.04(6)(f)] Observed potentially hazardous food items being held on front line, not stored at 135F or higher. CA: all side items were reheated to 165F in microwave since they were out of temperature for less than 4 hours. All chicken and shrimp was discarded.  Corrected On-Site.  New Violation.
11D Violation of Code: [511-6-1.05(3)(g)] Observed no thermometers in fryer area for employees to take temperatures of cooked foods. CA: Discussed with PIC. Thermometers will be provided and a training will be conducted.  New Violation.
Remarks Facility must conduct a training with all employees within 1 week. Proof of training should be submitted to the Health Department and all employees should sign stating they received and understood training. Facility will receive a re-inspection once all trainings have been completed with managers and employees.
