4-1A Violation of Code: [511-6-1.04(4)(c)1(i)(ii)(iii)(v)(vi)(vii)(viii)] Observed improper vertical separation of raw animal foods and ready-to-eat foods. Observed raw hamburger stored over fully cooked gyro meat in reach in freezer CA: PIC re-arranged freezer, PIC was educated on proper storage of RTE foods and raw foods in freezer  Corrected On-Site.  New Violation.
6-1A Violation of Code: [511-6-1.04(6)(f)] Observed potentially hazardous food cold held at greater than 41 degrees Fahrenheit. Observed foods on main cook line prep table at 43-47 degrees CA: PIC discarded all TCS foods and contacted cooler repair man, cooler will not be used until checked by health department  Corrected On-Site.  Repeat Violation.
12A Violation of Code: [511-6-1.04(4)(z)] Observed food labels that had fallen into prepped products in walk in cooler CA: PIC removed label, educated PIC options to use a grease marker to label foods to eliminate this problem  Corrected On-Site.  New Violation.
12B Violation of Code: [511-6-1.03(5)(i)] Observed employee with no hair restraint. CA: PIC put on hat  Corrected On-Site.  New Violation.
12C Violation of Code: [511-6-1.04(4)(m)] Wet wiping cloth not stored in sanitizing solution between uses. Observed multiple wet cloths not stored in sanitizer CA: PIC placed cloths in sanitizer  Corrected On-Site.  New Violation.
17C Violation of Code: [511-6-1.07(5)(a),(b)] Observed floors and walls not clean to sight and touch, observed build up of debris around coolers and fryers CA: PIC will have floors and walls cleaned prior to reinspection  Corrected On-Site.  New Violation.
