4-1A Violation of Code: [511-6-1.04(4)(c)1(i)(ii)(iii)(v)(vi)(vii)(viii)] Observed raw chicken batter stored directly on top of slaw and cooked rice at walk-in cooler.
Corrective Action: CFSM was notified about the issue, CFSM rearranged the food items.  Corrected On-Site.  New Violation.
4-2B Violation of Code: [511-6-1.05(7)(a)1] Observed multiple stacks of food pans and lids stored as clean with dried chicken batter.
Corrective Action: CFSM was notified about the issue, employee was instructed to re-wash rinse and sanitize the food pans and lids.  Corrected On-Site.  Repeat Violation.
4-2B Violation of Code: [511-6-1.05(7)(b)] Observed food and debris build-up on the contact surface of the bread warmer.
Corrective Action: CFSM was notified about the issue, CFSM assigned employee to clean the bread warmer.  New Violation. Correct By: 07/24/2019
6-1A Violation of Code: [511-6-1.04(6)(f)] Observed improper cold holding of TCS food items above 41 degrees in ice bath. Raw chicken 45 degrees, raw chicken 43 degrees, liquid batter 45 degrees, 44 degrees.
Corrective Action: CFSM was notified about the issue, food items were out less than 2 hours per CFSM. Food items were placed in walk-in cooler. Ice bath needed more ice and water. Employee refilled ice bath with ice and water.  Corrected On-Site.  New Violation.
12B Violation of Code: [511-6-1.03(5)(i)] Observed food service employee preparing sandwich without a hair restraint.
Corrective Action: Employee put on hair net and washed hands before returning to food preparation.  Corrected On-Site.  New Violation.
14A Violation of Code: [511-6-1.04(4)(k)] Observed in-use scoops stored in standing water less than 135 degrees with food debris build-up.
Corrective Action: CFSM was notified about the issue, scoops were washed, rinsed and sanitized and were placed under running water.  Corrected On-Site.  New Violation.
15A Violation of Code: [511-6-1.05(6)(a)] Observed 3 reach-in units (coolers and freezer), bread warmer and microwave oven inoperable.
Corrective Action: CFSM was notified about the issue, work order has been placed per CFSM. Item will be re-checked.  New Violation. Correct By: 07/26/2019
