1-2A Violation of Code: [511-6-1.03(1)(a)-(c)1,4,5,6-14,15, 16] Observed CFSM not performing duties by ensuring all food workers are maintaining safe food handling practices and guidelines as required by state regulations. Personnel will be trained, re-trained on all food safety guidelines. This will be rechecked  New Violation.
1-2B Violation of Code: [511-6-1-.03(3)(d)] Observed CFSM not performing duties by ensuring all food workers are maintaining safe food handling practices and guidelines as required by state regulations. Personnel will be trained, re-trained on all food safety guidelines. This will be rechecked  New Violation.
2-2A Violation of Code: [511-6-1.03(1)(c)17] Observed no evidence of employee health policy or acknowledgement of trained employees during inspection.
Corrective Action: CFSM was notified. Employee Health Policy will be provided and all food workers will sign acknowledgement of training. This will be rechecked.  Repeat Violation. Correct By: 07/12/2019
4-1A Violation of Code: [511-6-1.04(4)(c)1(i)(ii)(iii)(v)(vi)(vii)(viii)] Observed cookline employee handling container of raw chicken, then handling utensils for stirring food at cookline without changing gloves or washing hands.
Corrective Action: CFSM was notified. Employee was stopped immediately and instructed to change gloves and wash hands. All contaminated utensils were taken to dishwashing to be properly cleaned and sanitized.  Corrected On-Site.  New Violation.
6-1A Violation of Code: [511-6-1.04(6)(f)] Improper cold holding of potentially hazardous, TCS foods above 41 degrees observed during inspection. Raw Shrimp- 52 degrees and Raw Chicken- 51 degrees in ice bath station at cookline.
Corrective Action: CFSM was notified. All out of temperature food items were found to be less than 4 hours old and were placed in freezer for rapid cooling to 41 degrees.  Corrected On-Site.  Repeat Violation.
6-1B Violation of Code: [511-6-1.04(6)(f)] Observed improper hot holding of potentially hazardous, TCS foods during inspection. Cheese Dip – 130 degrees on cookline steam table
Corrective Action: CFSM was notified. Cheese dip was found to be less than 1 hour old and was properly reheated to 165 degrees prior to being placed back on steam table.  Corrected On-Site.  New Violation.
6-2 Violation of Code: [511-6-1.04(6)(g)] Observed several refrigerated, TCS foods held greater than 24 hours that were not date marked. Cooked Beans and tamales without date marking observed in front line reach-in cooler.
Corrective Action: CFSM discarded all undated food items  Corrected On-Site.  New Violation.
6-2 Violation of Code: [511-6-1.04(6)(h)] Observed several expired date marked TCS foods in facility. Rellenos Dated: 6/28/19 were still in frontline reach-in cooler after over 7 days. Observed Enchilada sauce dated: 6/28/19 still in walk-n cooler after over 7 days.
Corrective Action: CFSM was notified. All expired foods were discarded.  Corrected On-Site.  New Violation.
11A Violation of Code: [511-6-1.04(6)(e)] Observed salsa cooling in deep, covered containers greater than 12 inches in walk-in cooler. Salsa measured at 61 degrees
Corrective Action: Discussed proper cooling methods with CFSM and separating food items to be cooled in smaller portions and shallow containers to ensure proper cooling to 41 degrees.  Corrected On-Site.  New Violation.
12A Observed ice bins open at front kitchen area and bar area during inspection.
Corrective Action: CFSM was notified. All ice bins were properly covered  Corrected On-Site.  New Violation.
12C Violation of Code: [511-6-1.04(4)(m)] Observed 1 of 2 sanitizer buckets in kitchen area with no sanitizer present. 0 ppm chlorine.
Corrective Action: CFSM was notified. Sanitizer bucket was changed out and adjusted to 100ppm chlorine as required.  Corrected On-Site.  New Violation.
12C Violation of Code: [511-6-1.04(4)(m)] Observed sanitizer bucket stored directly on the floor in kitchen during inspection.
Corrective Action: CFSM was notified and relocated bucket off the floor as required  Corrected On-Site.  New Violation.
