1-2A Violation of Code: [511-6-1.03(2)(a)-(l)(n)(o)] PIC is unable to ensure compliance of the following: proper cooling methods and cooking techniques such as cold and hot holding. CA: PIC needs training.  New Violation. Correct By: 03/08/2019
1-2B Violation of Code: [511-6-1.03(3)(a)] Was unable to verify a ServSafe for the facility. CA: Facility has one month to receive a valid ServSafe or permit will be suspended. PIC will enroll in a ServSafe within two weeks, notify the Health Department, and provide written documentation.  New Violation.
4-1A Violation of Code: [511-6-1.04(4)(c)] Observed raw chicken stored in walk-in cooler touching raw beef. Also observed raw bacon stored touching raw beef and raw eggs stored above RTE foods. CA: PIC rearranged foods according to cook temperatures. Educated PIC.  Corrected On-Site.  New Violation.
6-1A Violation of Code: [511-6-1.04(6)(f)] Observed potentially hazardous foods not stored at 41°F or below (sliced tomatoes: 57°F, salsa: 58°F, chives: 57°F). CA: PIC rapidly cooled items to 41°F since they were stored out of temperature for less than 4 hours.  Corrected On-Site.  New Violation.
6-1B Violation of Code: [511-6-1.04(6)(f)] Observed potentially hazardous pork not properly hot held at 135°F or above (pork: 60°F). CA: PIC placed pork on grill to rapidly reheat food to 165°F since food was stored out of temperature for less than 4 hours.  Corrected On-Site.  New Violation.
6-1C Violation of Code: [511-6-1.04(6)(d)] Observed potentially hazardous beans not cooled within given parameters: 135°F-70°F within 2 hours and 70-41°F within 4 hours. Beans were 50°F. PIC stated that beans were cooked last night. CA: PIC discarded beans. Discussed proper cooling procedures.  Corrected On-Site.  Repeat Violation.
6-2 Violation of Code: [511-6-1.04(6)(g)] Observed potentially hazardous milk used for prep after its expiration date (02/24/19). CA: PIC discarded milk.  Corrected On-Site.  New Violation.
12C Violation of Code: [511-6-1.04(4)(m)] Observed wet wiping cloth not stored in sanitizer solution between uses. CA: PIC placed wet wiping cloth in sanitizer bucket.  Corrected On-Site.  New Violation.
17D Violation of Code: [511-6-1.05(2)(n)] Observed vent hoods not cleaned at a frequency necessary to be maintained cleaned to sight and touch. CA: PIC will clean vent hoods ASAP.  New Violation. Correct By: 03/08/2019
